I microwave for 2 min and for the first 10 minutes it produces a strong heat such as heating pad on high. Then for the next 10 minutes it produces a moderate heat that is still helpful. I hold the long ends so the middle sags down (so most of the material moves into the part that will cover the back and sides of my neck) and then fold the long ends under the wider neck region and then place it in the microwave (ends underneath). The high neck piece that tapers around the neck is perfect to cover the entire neck area - like a high, thick turtleneck. I've tried 2.25 minutes but it gets a little to hot for me to go directly on bare skin. Sometimes I put it back in the microwave (only for one minute) after I use it for 10 minutes to keep the heat on "high". Pulling my zipper hoodie (or a towel) over it does help intensify (at the beginning) or hold the heat for a little while longer. The velcro is a godsend to hold it in place and being able to move around and the fabric is super comfortable. Has no smell coming out of the microwave. And I've experience no "burning" of the fabric after many uses. I use it constantly, worth every penny for my neck pain!
